REQUIRING ALL OFFICIALS AND EMPLOYEES IN THIS OFFICE TO ACCOMPLISH THE ATTACHED FORMS.
In order to complete the personnel records in this Office, all officials and employees in this Office are hereby required to accomplish the attached forms.
The forms should be submitted to the Administrative Office not later than August 3, 1977.
