AMENDED SCHEDULE OF FEES AND CHARGES OF THE COOPERATIVE DEVELOPMENT AUTHORITY
Hereunder are the amended schedule of fees to be imposed and collected by the Authority. The highlighted applications below amends the CDA Memorandum Circular No. 02-03 dated April 23, 2002.
I - REGISTRATION APPLICATIONS FEES
A. Initial The initial registration fee to be imposed shall be 1/10 of 1% of the authorized share capital or the basic fee below whichever is higher.
a. Primary Co-ops Regular Lane Php 500.00 Express Lane 1,000.00 Verification Fee 50.00 b. Secondary Co-ops 2,000.00 c. Tertiary Co-ops 3,000.00 d. Laboratory Co-ops 50.00 B. Amendments a. On specific provision/s Php 300.00 b. By substitution 300.00 c. On the increase of capital 1/10 of 1% of the increased authorized share capital plus Php300.00 basic fee d. Merger Php 400.00 If there is increase in capital apply I(A) above e. Consolidation apply 1(A) above f. Division apply 1(A) above II - FORMS/LISTINGS a. Articles of Cooperation and By-laws, Php 50.00 (1 Economic Survey (diskette copying only) diskette) b. List of Co-ops (diskette or 1 RW CD Php 50.00/ copying only) diskette or Php 500.00 (1 RW CD) c. Printed copies of II (b) above Php 5.00/page III - FILING FEE a. Filing fee for complaint/petition, third- Php 300.00 party complaint and complaint or petition in-intervention b. Appeal Fee Php 300.00 IV – MISCELLANEOUS a. Certification Php 100.00 b. Certified True Copy 10.00/page c. Certified Xerox Copy 5.00/page d. Transcript of Stenographic Notes Php 6.00/page V - EFFECTIVITYThis Revised Schedule of Fees shall take effect fifteen (15) days after the date of receipt by the Office of the National Administrative Register (ONAR), University of the Philippines, Law Center.
